Cancellation Request Form

This form is for the use of incoming Freshmen and Transfer students wishing to cancel their Admission and/or Housing Agreement.

Current students wishing to withdraw from the University should contact Student Records. Current residents wishing to cancel their Housing Agreement should contact Housing Services.

Please complete the following form to submit your cancellation request.

Please note that this form replaces the Admitted Student Response Form that was included with your acceptance packet. If you have completed this online form to decline your offer of admission, you should not return the paper form.

If you are declining your offer of admission and have previously applied for on-campus housing, please note that your Housing Agreement will be cancelled, but you are subject to the cancellation policy as outlined in the Terms and Conditions of the Housing Agreement.  Please note that declining your offer of admission will also cancel any financial aid and scholarship offers. A copy of the Terms and Conditions of the Housing Agreement can be found on the Campus Housing page within Campus Connection.

By clicking "Submit" below, you have formally completed your cancellation with DePaul University. You will receive confirmation from the indicated offices via e-mail within two business days.
